We acknowledge that each award is different, but please try to include all of the items below that apply. If you have other content or information that you think may help us share the story of our work together, please include it as well.

  • Name or title of the group, organization or individual that best represents the recipient of the award.

  • Logos for recipients and/or involved organizations or groups.

    - File types: .jpg, .png
    - Dimensions: 150px minimum width

  • URLs of websites and/or other online properties of recipients.

  • Description and details of what the award will fund—listing focus, goals and targets for the award, along with immediate and broader expected impacts.

  • URLs of press releases and other online publications associated with the award.

  • Screenshot or images of any press releases or publications associated with the award.

    - File types: .jpg, .png
    -Dimensions: 800px to 2000px wide

  • PDF documents associated with the award, other relevant files or any other high-quality images capturing the essence of the work being done.

  • Name, email and phone number of who we should contact if we have questions on the submitted content.
